What is Urgent Community Response?
Urgent Community Response (UCR) is a service that supports patients either already in their own home or coming out of hospital following an admission. There are a number of different conditions that the UCR team can support you with.
Our team is made up of Physiotherapists, Occupational therapists, Assistants Practitioners, Nurses and Support assistants.
The different professionals you might see visiting you all wear different uniforms:
- Our Nurses either wear pale blue or navy uniforms
- Our Physiotherapists and Occupational Therapists wear white uniforms with a coloured trim (green or blue)
- Our assistant practitioners and support assistants wear a Teal coloured uniforms with or without a white trim.
What we do?
Once you have been referred by your Health care professional who is dealing with your care a clinician will visit you at home and assess your health needs. A treatment plan will be agreed with you.
Visits from our team will be scheduled and we will support you to improve your health and get you over your urgent health need. The team will visit you within 2 hours of referral. The team can see you for up to 3 days, sometimes longer if needed. Some of our patients need seeing multiple times per day to keep them well and out of hospital.

Chaperone
If you feel you would like a chaperone, please discuss with your visiting nurse prior to the procedure. To accommodate this your visit may need to be rescheduled. A carer, friend or relative is welcome to support you during procedures.

This service is based at The James Cook University Hospital but also covers the Friarage Hospital in Northallerton, our community hospitals and community health services.
