Work is generated in GP practices within the Redcar and East Cleveland area.
The aim of the service is to provide initial assessment to all patients with new coronary heart disease diagnosis and to those who are new to practice.
The service also provides annual assessments to all patients on Ischaemic Heart Disease Register and follow up care based on patient need.
The service facilitates a proactive approach to reducing individual coronary heart disease risk factors, burden of disease and reduces unnecessary hospital admissions.
Referrals are made from GPs, and occasionally from the acute teams.
The team are operating a telephone triage in the first instance, where necessary a video or face to face consultation will take place and following that where appropriate a face to face visit will occur.
