We aim to:
- Support you at home on discharge from hospital
- Prevent you having to be admitted to hospital
This service is for adults who are usually unable to attend outpatient appointments.
Following assessment, we may make regular visits to work with you on an agreed treatment plan.
Our team consists of a physiotherapist, an occupational therapist and three rehabilitation assistants.
We work with older people and other adults with rehabilitation needs, eg following a stroke. We also help to support families and carers with advice and education.
What happens during an appointment?
At the first visit, you will be assessed by the most appropriately qualified therapist. Should you require further treatment, this will be explained and planned with you.
Further visits may be made by a team member who will work with you according to the agreed treatment plan.

The team currently offers a five day service Monday to Friday from 8.30am to 4.30pm.
How to refer?
Initial referral is via your GP or other health and social care professional, such as a hospital consultant, social worker or community matron.
