We provide respiratory education, recommendations and home visits to patients with complex respiratory needs.
Our patients are referred by their health professionals and GPs and the team also carries out training in community settings and Teesside University, working closely with our commissioners and visiting patients with complex breathing problems at home.
We work with patients, carers, all levels of health professionals and commissioners.
Who do we work with?
We work with patients, carers and all levels of health professionals and commissioners (GPs and clinical commissioning groups) who buy their services from the trust).
What happens during an appointment?
Once a patient has been referred by a health professional, the team will visit you at home and carry out a complete respiratory assessment including breathing tests and checking on oxygen levels before the appropriate treatment is prescribed.
Staff will also discuss a comprehensive education package with patients to help them manage their condition more effectively and refer people to any other appropriate agencies if necessary, writing a detailed letter to both the referring health professional and GP (if different). The team will also leave you contact details for future reference.
