The palliative and end of life care champions are made up of acute nurses, hospice nurses, community nurses, allied healthcare professionals and chaplains, to promote palliative and end of life care within their own clinical area or departments and act as a link to the specialist palliative care team.
The champions will work with their clinical teams to develop best practice in palliative and end of life care whether this is in the acute hospital environment, the community or primary care hospitals.
They have undertaken this in a variety of ways; including the development of palliative and end of life care poster boards in their teams to share information with their colleagues.
Through the champions, we aim to provide patient and family centred holistic palliative care – keeping patients at the centre of what we do.

Chris Ward is clinical educator in palliative care for the trust. Chris recruits and supports the champions in partnership with the trust’s specialist palliative care team.
